IMPORTANT NOTE: Only the first 32 players to register and provide payment (per team) will be placed on the roster – subsequent player registrations will be placed on a waitlist and are not guaranteed a spot on the team

--> Form: Medical Clearance

--> Form: Waiver and Release of Liability

--> Form: Image Release

--> Form: Emergency Medical Treatment, Consent and Information

--> Copy of birth certificate or passport.

8th Grade Unlimited - in addition to items above

--> Copy of player’s 2011-12 report card (for verification of 9th grade eligibility)

Financial Assistance

---> For those financially unable to make a full payment, financial assistance arrangements and associated partial payments must be completed by June 30. Limited funds are available for Financial Assistance.  Contact paloaltoknights@gmail.com for additional information.

--> All forms and copies (Medical, Liability, Image release, Emergency Contact, Birth Certificate, 8th grade report card) due by June 30.
